Never Events are the sort of events that should never happen within healthcare. Wrong site surgeries, lumbar punctures being performed on the wrong patient, wrong implants used, oral medication given intravenously. Thankfully most never events are rare on a population basis, however when they happen they can be truly devastating to patients.
Because never events are largely preventable, we are all under increased pressure to eliminate them and to learn from incidents made elsewhere.
NHS Improvement publicly report Never Events not to place blame upon organisations when these incidents do occur but rather to learn from what happened, to help others identify potential weaknesses in how an organisation manages fundamental safety processes and to improve the quality of care. From 1st April to 31 July 2018 there were 150 Never Events reported across the UK.
